The attachment plates on the side have bottom hole trimmed off and for adjusting height of lower brackets were also cut. Only the two circled in blue are usable. This will not work on a truck that sits high off the ground. Here is a used Western Ultramount to Unimount adapter 64520.
See the pictures to see what configurations are usable. Only two of the original 8 configurations are still available to be used. The last 6 pictures are for reference only. The con rations with the blue X through them are not usable.
Buyer needs to do their own research before buying. This adapter connects into a Western Ultramount truck mount. There was one previous version of this adapter that stuck too far out and apparently did not hold up well. This was done because the adapter previously on a half-ton Chevy that didn't sit very high off ground, so lower holes were not used cut to increase ground clearance.
Having those holes cut off limits the height adjustment of this adapter quite a bit. The Western Ultramount truck mount should already be installed on a to figure out what height the adapter with sit at see last 6 pictures get an idea of how calculate correct for. Adapter is in very good overall condition. Included is the what you see in pictures.
